Career Pathways Program Services & Required Forms
What can the Career Pathways program offer parents?
Students enrolled in the Career Pathways program are eligible for the following financial and non-financial services:

| Academic Advising | Uniforms |
| Help finding employment | Program supplies & equipment |
| Help enrolling in classes | Childcare |
| Help applying for financial aid & scholarships | Transportation |
Required Forms for Enrolled Students/Participants:
- Career Pathways Application w/Instructions
- Class Attendance Verification – Allied Health Students
- Class Attendance Verification – All Other Students
- Employment Verification Form (for those who do not receive written documentation of earnings)
- Request for Financial Assistance
- Request for Transportation Assistance
- Request for Childcare Assistance
- Mid-Term Grade Report
- Student Update Form (to change address, contact and/or employment information)
- Annual Student Contract
- Career Pathways Handbook
- DWS Referral Form
